With evolving state policies and anticipated shifts in Medicaid funding, there is a unique opportunity to innovate and fortify our system of care. We are already witnessing the effects; several long-standing nonprofits have modified or suspended programs in response to changing reimbursement structures.
The San Mateo County Behavioral Health Advocates are devoted community members focused on strengthening the behavioral health system and enhancing outcomes for those it serves. Our mission is to initiate and fund a comprehensive, evidence-based analysis of the system’s needs and gaps. This will help empower us to craft forward-thinking proposals that will guide the future of behavioral health services in San Mateo County.
We’ll provide an outline of the objectives, scope, and methodologies for an exhaustive review of behavioral health services in San Mateo County. Our goal is to equip local leaders with a detailed understanding of provider roles, unmet needs, and opportunities for creating a more robust, sustainable system.
Our review aims to inform County decision-makers, nonprofit leaders, and funders, enabling them to make well-founded choices regarding services and investments. We will also be identifying new opportunities for nonprofit providers to fill existing gaps in care. We will seek the engagement of relevant stakeholders in San Mateo County to support funding for this assessment.
Mapping the Path Forward
Assessment Phases
In the first phase, we will focus on identifying existing service gaps, particularly those programs that have been scaled back or discontinued under CalAIM. We will analyze the cost and staffing realities of County-operated versus nonprofit clinics.
Our analysis will leverage comparative data from peer counties, including Medicaid usage rates—where San Mateo County currently reports one of the lowest in the state—per-capita spending for behavioral health and substance use disorders (SUDs), and innovative service delivery strategies in other jurisdictions.
Phase One: Current Service Gaps
The second phase will analyze the potential impacts of impending federal Medicaid reductions and subsequent state budget adjustments, anticipated to follow the federal fiscal year starting October 1, 2025. This phase will identify services at risk of being reduced or eliminated and outline alternative delivery models, including the establishment of new nonprofits, to ensure access and continuity of care.
